How to refer:
1. Our referral form is integrated with Ontario’s most commonly used EMRs:

1. From the start menu, select Tools > Form Editor
2. A new box will pop up. Select Tools in top left corner and then Publish / Download
3. Ensure you’re on the Download Forms tab, and the Category is Standard Forms
4. Search “MindSeed”. Select the form “MindSeed Referral Form” and click Download

1. Click Help on top > External Resources > PS Suite Community Portal
2. In the forms library, look for “MindSeed Referral” within the “Ontario” province
3. Download the form onto to your computer
4. From the PS Suite Settings menu, choose Edit Custom Forms
5. Drag and drop the file you downloaded (it will be a .cfm file) into the list of custom forms; OR in the Forms window click File > Import Form > select the file
6. When you want to use the form for a patient, click Data then New Custom Form to insert it into their chart

2. Search for “MindSeed Health Referral Form”
3. Click “Download” under “Action”
4. To Import the Zip file you just downloaded, go into your OSCAR System > Administration Panel > Manage eForms > Import > Choose File > Import
If your organization is integrated with Ocean, our referral form is also available on the Ocean Healthmap.
2. Download our fillable referral form and fax it to 416-223-5913 or email to info@mindseedhealth.com.
3. Fax your own Consultation Request form to 416-223-5913.
